The following is a tool to help guide you through the application process. Please check-off the steps as you complete them (this page does NOT need to be submitted as part of your application packet). All Classes Are Held 8 a.m. to 5 p.m. Monday to Friday

February 2016
Monday to Friday –February 8 to February 12 (first week)
Monday to Friday – February 15 to February 19 (second week)

April 2016
Monday to Friday – April 11 to April 15 (first week)
Monday to Friday – April 18 to April 22 (second week)

June 2016
Monday to Friday – June 13 to June 17 (first week)
Monday to Friday – June 20 to June 24 (second week)

August 2016
Monday to Friday – August 1 to August 5 (first week)
Monday to Friday – August 8 to August 12 (second week)

October 2016
Monday to Friday – October 10 to October 14 (first week)
Monday to Friday – October 17 to October 21 (second week)

December 2016
Monday to Friday – December 5 to December 9 (first week)
Monday to Friday – December 12 to December 16 (second week)

Tuition: $3,200.00
Check, money order made out to OhioHealth Sleep Services or a credit card by phone are acceptable forms of payment. You can also pay online by clicking here. A non-refundable application fee of $25 will be required when application is submitted. The remaining balance will be due by one week prior to start of the course.

Checklist for Enrollment Application:

  1. Application: The application should be completed by the applicant for admission
  2. Copy of High School Diploma: A copy of your high school or college diploma must be submitted
  3. Resume: A resume is required
  4. Application fee: A $25 non-refundable application fee
  5. Statement of Purpose: Submit a typed, double-spaced essay, minimum 250 words
